This expansion to Pandemic includes new event cards, new role cards, rules for five players, and optional game challenges to increase the difficulty, such as the 'Legendary' difficulty level, the Virulent Strain challenge, the Mutation challenge, and the Bio-Terrorist challenge. This is one board game where I would *absolutely* recommend the expansion, 'On The Brink'. The game does get a little stale without it, but the expansion provides another level of difficulty and unpredictability.
One thing to watch out for in this game is an overly-dominating personality in one of your fellow players. The only bad games I've ever had are those where one person is just telling everyone else what to do - this sucks all the co-operation and fun out of the game quickly. Play with a good group and you will love Pandemic.
Before I go any further, first let me clarify that you must have at least some version of the base game in order to play. On The Brink is not a stand-alone game and does nothing for you without the original Pandemic.
This expansion adds new challenges and twists, new roles, new events, and three new variations in the base game to allow you to change it up. It builds off of everything that is great about the original game by adding new layers and new interest. My friends and I play Pandemic over and over, and we almost never play anymore without at least using some part of On The Brink (roles, events, or rules). In fact, we play it so much I had to order replacement card decks because my original decks got so beat up. I learned my lesson and bought clear card-covers for the second set of decks to keep them looking new for future expansions!
If you're only going to buy one expansion for Pandemic, make it this one. If you're going to buy more expansions after that, I'd recommend "State of Emergency" next, and then "In The Lab" last.
This expansion actually adds four challenges: the mutation challenge (adds a 5th disease to the board), the virulent strain challenge (which adds new behaviours and increased complexity to the original 4 diseases), a bio-terrorist challenge that pits one player against the rest of the players, and it also adds a 7th regular epidemic card that allows you to play the original, base game with 7 epidemics.
