Extremis did a few things successfully. Firstly, it displayed the character of Tony Stark in a way that was interesting and accessible to new readers. Tony is a man who hates himself for the things he's had to do to get where he is. One of the points of the story is him trying to come to terms with who he is and his impact on the world. This was well portrayed without being overbearing. Second, they played with the military vs humanitarian applications of technology as a overarching theme of the story.
